Effective June 1, 2021
IESA/IHSA has released a new physical form w/COVID Supplemental which will affect any student who plans to participate in athletics at MMS or MHS for the 2021-22 school year.
MHS Freshmen who submit a 9th grade physical will need pages 1 & 2 completed & submitted to the Athletic Office as well.
THIS WILL NOT AFFECT YOU IF you have already had a physical prior to June 1 or submitted your physical prior to June 1. The school nurse may require additional information as new guidelines are issued.
